Roster Template for Directory Update

Embright has a responsibility to provide its members with accurate provider directory information. To do so, we rely on providers to notify Embright if they come across any inaccurate information in the provider directory, so that timely and appropriate corrections can be made. 

If you come across any inconsistencies or important changes in the directory information, we kindly request that you take a moment to complete the roster template and send it to providersupport@embright.com. By doing so, you can ensure that any necessary modifications are made and that your practice and clinician information stays accurate and up to date.
